A Bracknell woman did not mean to kill herself when she deliberately took a lethal overdose of heart medication, a coroner has ruled.

Sammy Joe Williams died after overdosing at her Bracknell flat on October 27, 2021, before passing away one day later at Royal Berkshire Hospital.

After being taken to hospital, the 22-year-old told doctors: ‘I wish I had not done it, now.’

Reading Coroner’s Court heard how Sammy, who suffered from PTSD, depression and emotionally unstable personality disorder, had a history of drug overdoses and suicide attempts dating back to May 2014.

The latest overdose came two days after a family argument at a caravan park in Devon.

A male relative had been arrested following an alleged assault against Sammy at the caravan park on October 25, an accusation the suspect denied.

Although Reading Coroner's Court heard about the incident, the coroner did not draw a direct link to Sammy's overdose.

Sammy, a carer, subsequently travelled back to Bracknell from Devon and took an overdose of heart medication after calling her mother to tell her she wanted to end her life.

Her mother Belinda called a relative to check on Sammy, who was found after having taken an overdose at her flat just before 1am on October 27.

Despite extensive treatment lasting almost two days, Sammy died at Royal Berkshire hospital surrounded by family at 5 pm on October 28 after suffering multiple organ failures.

Berkshire coroner Alison McCormick said: “It is clear from the evidence that although she deliberately took the overdose and she did so having told her mother she intended to end her life, when she was in hospital she expressed regret and she did not appreciate the toxicity of what she had taken.

“It seems to me that the taking of an overdose was an impulsive act against the background of her mental illness and perhaps as a reference to the incident that occurred in Devon.

“The overdose of medication was deliberately taken but I’m not able to say it is likely she intended to end her life.”

Ms McCormick opted against concluding that Sammy died by suicide, and instead stated that the 22-year-old’s intention was ‘unclear.’

The coroner addressed Belinda at an inquest into her daughter’s death.

She said: “I’m so sorry that you lost Sammy in this way. It must have been a very difficult and traumatic experience for all of you.

“It seems to me you gave Sammy as much support and love as you could and were always there for her. There was nothing you could have done.”

The inquest was heard at Reading Coroner’s Court on Monday, August 15.
