One might assume the dog was locked in. Many disabled toilets use what I believe is called a "radar key" to prevent casual access, therefore it is possible for someone with said key to lock their dog in the toilet. Why on earth anyone would do that is another matter...

As I don't carry one of those keys around, if I heard a dog yelping inside a disabled toilet I would probably call the fire service myself, not least because for all I know the owner could be laying unconscious on the floor!
